Correct: Acute appendicitis
The appendix looks normal (Image 1 and 4), except the distal portion which is edematous and with loss of wall stratification (Image 2 and 3). The aspect can also be seen on 3D, the distal appendix is dilated and with a hypoechoic wall. The appendiceal wall has also an intense Doppler signal.
